UK – APHA part of 5 UK E. coli outbreaks in 2022

The Animal and Plant Health Agency (APHA) was involved in another two E. coli outbreaks in the United Kingdom in the final quarter of 2022.

In the first, APHA helped Public Health Wales investigate two human cases of Shiga toxin-producing E. coli (STEC) O145 linked to a private collection of animals on a smallholding. Multiple species were onsite including cattle, goats, deer, and pigs. Some food consumed by people was grown in the garden where cattle manure was used. Thirty environmental fecal samples were collected but a match to the outbreak strain was not found.

In the second, APHA visited an open farm at the request of an incident management team following an outbreak of E. coli O157 in people. The outbreak strain was detected in one environmental sample from a pig enclosure. The incident is ongoing so it is unclear how many people are sick. The advice was provided to reduce the risk to the public by making improvements to the supervision of animal contact, enhancing handwashing facilities, and improving some animal exhibits.

In all of 2022, APHA was part of five E. coli investigations. The agency helped the UK Health Security Agency (UKHSA) investigate E. coli O103, O145, and O26 outbreaks between July and September.

The E. coli O26 outbreak also involved cryptosporidium. There were 11 cases of cryptosporidium and two people had confirmed E. coli O26. Cryptosporidium patients had visited an open farm attraction during the incubation period of illness. The E. coli patients had links to the same premises.

The E. coli O103 outbreak with 11 cases was associated with soft, raw milk cheese from a dairy farm in the East of England. Pasteurization was put in place for the production of the soft cheese, HACCP processes were reviewed, and enhanced control measures were taken.

The E. coli O145 outbreak with 10 patients was traced to the consumption of milk products from a dairy farm in North West England, with illness onset from mid-July. Investigations identified an issue with pasteurization and problems with the cleaning and storage of milk crates.

DUMAGUETE CITY – At least nine water sources from different areas in Vallehermoso town, Negros Oriental province, tested positive for E.coli (Escherichia coli) bacteria that left five people dead, and more cases reported over the weekend.

Jennifer Remollo, the provincial officer of the Department of Health (DOH) in Negros Oriental, told the Philippine News Agency in an interview Monday that laboratory tests were done from water samples taken from these sources after the number of cases skyrocketed last week.

“As of today, our office has recorded 203 cases of acute gastroenteritis, including the five deaths that were earlier reported last week,” Remollo said.

“Consumption of E.coli contaminated water sources can be fatal due to severe dehydration from diarrhea,” she said.

Included in those that tested positive for the bacteria were mostly from deep well and spring sources in the areas of Barangay Don Espiridion Villegas; Sitio Sangi, Barangay Tabon; Sitio Patag, Barangay Puan; Sitio Tubod, Barangay Malangsa; Barangay Puan; Malapay 1, Barangay Bagawines;; Barangay Macapso; and Barangay Poblacion, Remollo said.

The DOH official said constant rains for many days in these areas may have caused the contamination of these water sources.

She said 82 of those who came down with symptoms were admitted to the hospital.

Those who died were residents of Barangay Puan.

Research -Burden of foodborne disease due to bacterial hazards associated with beef, dairy, poultry meat, and vegetables in Ethiopia and Burkina Faso, 2017

Foodborne disease is a significant global health problem, with low- and middle-income countries disproportionately affected. Given that most fresh animal and vegetable foods in LMICs are bought in informal food systems, much the burden of foodborne disease in LMIC is also linked to informal markets. Developing estimates of the national burden of foodborne disease and attribution to specific food products will inform decision-makers about the size of the problem and motivate action to mitigate risks and prevent illness. This study provides estimates for the burden of foodborne disease caused by selected hazards in two African countries (Burkina Faso and Ethiopia) and attribution to specific foods. Country-specific estimates of the burden of disease in 2010 for Campylobacter spp., enterotoxigenic Escherichia coli (ETEC), Shiga-toxin producing E. coli and non-typhoidal Salmonella enterica were obtained from WHO and updated to 2017 using data from the Global Burden of Disease study. Attribution data obtained from WHO were complemented with a dedicated Structured Expert Judgement study to estimate the burden attributable to specific foods. Monte Carlo simulation methods were used to propagate uncertainty. The burden of foodborne disease in the two countries in 2010 was largely similar to the burden in the region except for higher mortality and disability-adjusted life years (DALYs) due to Salmonella in Burkina Faso. In both countries, Campylobacter caused the largest number of cases, while Salmonella caused the largest number of deaths and DALYs. In Burkina Faso, the burden of Campylobacter and ETEC increased from 2010 to 2017, while the burden of Salmonella decreased. In Ethiopia, the burden of all hazards decreased. Mortality decreased relative to incidence in both countries. In both countries, the burden of poultry meat (in DALYs) was larger than the burden of vegetables. In Ethiopia, the burdens of beef and dairy were similar, and somewhat lower than the burden of vegetables. The burden of foodborne disease by the selected pathogens and foods in both countries was substantial. Uncertainty distributions around the estimates spanned several orders of magnitude. This reflects data limitations, as well as variability in the transmission and burden of foodborne disease associated with the pathogens considered.

Research – Treatment of Ready-To-Eat Cooked Meat Products with Cold Atmospheric Plasma to Inactivate Listeria and Escherichia coli

Abstract

Ready-to-eat meat products have been identified as a potential vehicle for Listeria monocytogenes. Postprocessing contamination (i.e., handling during portioning and packaging) can occur, and subsequent cold storage together with a demand for products with long shelf life can create a hazardous scenario. Good hygienic practice is augmented by intervention measures in controlling post-processing contamination. Among these interventions, the application of ‘cold atmospheric plasma’ (CAP) has gained interest. The reactive plasma species exert some antibacterial effect, but can also alter the food matrix. We studied the effect of CAP generated from air in a surface barrier discharge system (power densities 0.48 and 0.67 W/cm2) with an electrode-sample distance of 15 mm on sliced, cured, cooked ham and sausage (two brands each), veal pie, and calf liver pâté. Colour of samples was tested immediately before and after CAP exposure. CAP exposure for 5 min effectuated only minor colour changes (ΔE max. 2.7), due to a decrease in redness (a*), and in some cases, an increase in b*. A second set of samples was contaminated with Listeria (L.) monocytogenesL. innocua and E. coli and then exposed to CAP for 5 min. In cooked cured meats, CAP was more effective in inactivating E. coli (1 to 3 log cycles) than Listeria (from 0.2 to max. 1.5 log cycles). In (non-cured) veal pie and calf liver pâté that had been stored 24 h after CAP exposure, numbers of E. coli were not significantly reduced. Levels of Listeria were significantly reduced in veal pie that had been stored for 24 h (at a level of ca. 0.5 log cycles), but not in calf liver pâté. Antibacterial activity differed between but also within sample types, which requires further studies.

Research – Repeated cross-sectional study identifies differing risk factors associated with microbial contamination in common food products in the United Kingdom

Abstract

All foods carry microbes, many of which are harmless, but foods can also carry pathogens and/or microbial indicators of contamination. Limited information exists on the co-occurrence of microbes of food safety concern and the factors associated with their presence. Here, a population-based repeated cross-sectional design was used to determine the prevalence and co-occurrence of Escherichia coli, Klebsiella spp., Salmonella spp. and Vibrio spp. in key food commodities – chicken, pork, prawns, salmon and leafy greens. Prevalence in 1,369 food samples for these four target bacterial genera/species varied, while 25.6% of all samples had at least two of the target bacteria and eight different combinations of bacteria were observed as co-occurrence profiles in raw prawns. Imported frozen chicken was 6.4 times more likely to contain Salmonella than domestic chicken, and imported salmon was 5.5 times more likely to be contaminated with E. coli. Seasonality was significantly associated with E. coli and Klebsiella spp. contamination in leafy greens, with higher detection in summer and autumn. Moreover, the odds of Klebsiella spp. contamination were higher in summer in chicken and pork samples. These results provide insight on the bacterial species present on foods at retail, and identify factors associated with the presence of individual bacteria, which are highly relevant for food safety risk assessments and the design of surveillance programmes.

Abstract

Traditional cheeses are part of the Portuguese gastronomic identity, and raw milk of autochthonous species is a common primary ingredient. Here, we investigated the presence of Listeria monocytogenes, Coagulase Positive Staphylococci (CPS) and pathogenic Escherichia coli, as well as of indicator microorganisms (E. coli and other Listeria spp.) in 96 cured raw milk cheeses from the Alentejo region. Whole genome sequencing (WGS) of pathogenic E. coli and Listeria spp. as well as antimicrobial resistance (AMR) screening of E. coli isolates was also performed. L. monocytogenes, CPS > 10cfu/g and Extra intestinal E. coli were detected in 15.6%, 16.9% and 10.1% of the samples, respectively. Moreover, L. monocytogenes > 102 cfu/g and Staphylococcal enterotoxins were detected in 4.2% and 2.2% of the samples, respectively. AMR was observed in 27.3% of the E. coli isolates, six of which were multidrug resistant. WGS analysis unveiled clusters of high closely related isolates for both L. monocytogenes and L. innocua (often correlating with the cheese producer). This study can indicate poor hygiene practices during milk collection/preservation or during cheese-making procedures and handling, and highlights the need of more effective prevention and control measures and of multi-sectoral WGS data integration, in order to prevent and detect foodborne bacterial outbreaks.
