Guatemala – Four deaths in Guatemala linked to contaminated shellfish – Saitoxin – PSP

More than 30 people have fallen sick and four have died in Guatemala after eating shellfish.

The National Commission for the Surveillance and Control of Red Tide in Guatemala has extended an alert because of the presence of saxitoxins above the limits on the coasts of Tiquisate, Escuintla, Retalhuleu and San Marcos.

Public health officials said that since the end of April, 34 people had been affected and three children and one adult had died.

Authorities urged the public not to eat bivalve mollusks such as mussels, clams and oysters.

A warning was issued earlier this month after monitoring and analysis detected high concentrations of saxitoxins above the limits.

Saxitoxin is a toxin responsible for paralytic shellfish poisoning (PSP). Harmful algal blooms are often called red tides.

While most foodborne outbreaks are tracked through the FDA’s Coordinated Outbreak Response and Evaluation (CORE) network, seafood-related illnesses caused by natural toxins have a unique reporting mechanism. This web page provides information on commonly occurring seafood-related illnesses and how to report them to the FDA.  To report an illness from raw bivalve molluscan shellfish, email the FDA at shellfishepi@fda.hhs.gov.

The FDA receives reports of illnesses associated with the consumption of specific types of fish. These illnesses may originate from the activity of certain bacteria, toxins produced by marine algae, or hazards inherent in the fish.

Illness and reporting information for the commonly occurring illnesses are provided in each section. Contact the FDA with any questions or need for clarification of the illness or reporting.

Mycotoxins are defined as secondary fungal metabolites, toxic to humans and animals. Worldwide, mycotoxins have a significant impact on human and animal health, economies and international trade, making feed contamination by mycotoxins an area of great concern. However, how do mycotoxins relate to the immune system? In literature, there are several reports of mycotoxins negatively effecting it.

Animal immune systems are modulated during early life due to stimulus given through diet, microbiota colonisation and gastrointestinal mucosa. To what degree, and how, can mycotoxin exposure during early life modulate the immune system of young animals?

Mycotoxins that contaminate crops and animal feed have been recognized as a risk to farm animals, and account for considerable economic costs to the feed and food industries. Their widespread occurrence and related threat has been documented consistently in the BIOMIN Mycotoxin Survey.

Dangers to feed safety, sustainability

“Mycotoxins are among the most important safety risks for the future livestock feed industry and security of the feed supply chain,” stated Dr Gunther Antonissen of Ghent University in Belgium.

Fungi-produced mycotoxins endanger more than feed safety and security. They also hamper productivity, adding additional cost to the feed and food industry while also affecting the environment.

“Due to their negative effects on farm animal productivity and health, mycotoxins prevent the animal protein industry from achieving an efficient and sustainable use of natural resources,” observed Dr. Wulf-Dieter Moll of the BIOMIN Research Center.

Harmful mycotoxins do not have to contaminate feed in high concentrations to make their negative effects felt in farm animals. “At present, clinical mycotoxicosis caused by high doses is rare,” explained Dr Antonissen.

“However, also the ingestion of low to moderate levels of these toxins cause an array of metabolic, physiologic and immunologic disturbances, with the gastrointestinal tract as one of the major target organs,” he added.

Marine toxins originate from microorganisms native to aquatic ecosystems. These molecules eventually find their way into the human gastrointestinal tract through concentrating and bioaccumulating in species such as mollusks, crustaceans, and various fish. Ingestion of marine toxins can generate foodborne illnesses and a constellation of neurologic and gastrointestinal manifestations accompanied by other symptoms.

Ciguatoxin

Ciguatera illness is caused by ciguatoxins, which are compounds that bioaccumulate in shallow, coastal water-dwelling fish.

Saxitoxin

Paralytic shellfish poisoning (PSP) is a foodborne illness brought on by saxitoxin, a chemical compound produced by cyanobacteria of freshwater and by dinoflagellates of marine water. As with ciguatoxin, saxitoxin reaches the human gastrointestinal tract through concentration in species that are higher up in the food chain.

Brevetoxin

Also a result of toxic algal blooms and mollusk consumption, neurotoxic shellfish poisoning (NSP) is thought of as a ‘milder’ case of the paralytic shellfish poisoning described above. Its cause is brevetoxin, a group of more than 10 lipid soluble polyether compounds.

Tetrodotoxin

Tetrodotoxin (TTX) is perhaps the most well known of the marine toxins. Its notoriety arises from the popularity of pufferfish.

Histamine

A red herring in the recognition of fish food poisoning is scombroid syndrome. This illness is commonly mistaken for fish allergy, but instead results from improper storage and transportation of fish belonging to the Scombroidiae family.
