USA – Outbreak Investigation of Listeria monocytogenes – Hispanic-style Fresh and Soft Cheeses (February 2021)

The FDA, along with CDC and state and local partners, are investigating a multistate outbreak of Listeria monocytogenes infections potentially linked to Hispanic-style fresh and soft cheeses.

FDA has initiated an investigation and is collecting information to try to determine the source of the outbreak. Although the investigation is ongoing, CDC’s analysis of epidemiologic information indicates that Hispanic-style fresh and soft cheeses are the leading hypotheses for the cause of the illnesses. No specific type or brand of cheese has been confirmed as the source of illness. CDC reported that seven people infected with the outbreak strain of Listeria monocytogenes have been reported in Connecticut, Maryland, New York, and Virginia. Six of the seven ill people are Hispanic. Of the four people interviewed, three reported eating at least one type of Hispanic-style fresh and soft cheeses, and each of the three reported eating queso fresco.

As part of this outbreak investigation, the Connecticut Department of Public Health collected product samples of El Abuelito-brand Hispanic-style fresh and soft cheeses from a store where a sick person bought cheeses. Sample analysis showed the presence of Listeria monocytogenes in samples of El Abuelito Queso Fresco sold in 10 oz packages, marked as Lot A027 with an expiration date of 02/26/2021. Samples are currently undergoing Whole Genome Sequencing (WGS) analysis to determine if the Listeria monocytogenes found in these samples is a match to the outbreak strain. At this time, there is not enough evidence to determine if this outbreak is linked to El Abuelito Queso Fresco.

FDA and state partners are working with the firm to recall the affected lot and to determine if additional products should be recalled. Additional information will be provided as it becomes available.

Recommendations

Consumers, restaurants, and retailers should not eat, sell, or serve El Abuelito Queso Fresco cheese until more information is known.

If you are at higher risk for severe Listeria illness (if you are pregnant, aged 65 or older, or have a weakened immune system due to certain medical conditions or treatments) do not eat any Hispanic-style fresh and soft cheeses, including El Abuelito brand queso fresco cheese, until we identify which cheeses are making people sick in this outbreak.

Call your healthcare provider right away if you have these symptoms after eating Hispanic-style fresh and soft cheeses:

  •  Pregnant people typically experience only fever, fatigue, and muscle aches. However, Listeria infection during pregnancy can lead to miscarriage, stillbirth, premature delivery, or life-threatening infection of the newborn.
  •  People who are not pregnant may experience headache, stiff neck, confusion, loss of balance, and convulsions, in addition to fever and muscle aches.

Generally, make sure the Hispanic-style fresh and soft cheeses (like queso fresco) you eat have labels that state, “Made with pasteurized milk.”

Be aware that Hispanic-style fresh and soft cheeses made from pasteurized milk have caused Listeria outbreaks. Although pasteurization of milk kills Listeria, products made from pasteurized milk can still become contaminated if they are produced in facilities with unsanitary conditions.

Restaurants and retailers that serve or sell Hispanic-style fresh and soft cheeses should follow FDA’s safe handling and cleaning advice.

Lisa Fickenscher of the New York Post writes that an apparent salmonella outbreak at a resort in Jamaica in Dec. has ruined vacations for potentially dozens of holiday travelers — and now some sickened guests are considering legal action, The Post has learned.

“It was a nightmare,” Chantel Ele of Lincoln, Nebraska, told The Post of her experience at the Grand Palladium Resort and Spa, which was echoed by other people online. “I don’t know how many people go on an all-inclusive vacation and lose weight.”

Seven people in four states are sick, including Maryland, Connecticut, New York, and Virginia. This is not the first time a deadly outbreak has been linked to those types of soft and fresh cheeses.

These types of cheeses have long been a risk for Listeria monocytogenes contamination, according to the CDC. The issue is that many of these soft and fresh cheeses are made with unpasteurized milk. The government estimates that “the risk for listeriosis per serving is estimated to be 50- to 160-fold greater for cheese made from unpasteurized milk than pasteurized milk.”

Victims of foodborne illness are often so numerous that the government wants to cast the wide internet in contacting them about felony prosecutions that often occur years later.

Federal prosecutors who plan to take former Blue Bell Creameries president Paul Kruse to a jury trial in July are telling the judge this is just such a case. They filed a motion on Friday, Feb. 12, asking for a court order authorizing the Department of Justice (DOJ) to comply with the Crime Victims’ Rights Act “by using reasonable alternative measures for notifying potential unknown victims” in the Kruse case where the defendant is charged with multiple counts of fraud and conspiracy in relation to a deadly Listeria outbreak

Finland – Salmonella sickens 30 in Finland

At least 30 people have fallen ill after eating at a restaurant in Finland during the end of January.

An investigation by public health officials in the town of Kokkola, also known as Karleby, found Salmonella was behind the illnesses.

The first infection was recorded on Jan. 27 and no patients have been reported since Jan. 29. The most common symptoms include diarrhea, fever and abdominal pain.

Sigrid’s restaurant carried out cleaning and extensive disinfection after the suspicion of an outbreak was raised. Operations at the eatery are continuing as normal.

Frozen product suspected
Initially, no Salmonella was found in food and surface samples. Additional samples were taken in the restaurant, from staff and of the food.

Preliminary results show the suspected source of infection is a frozen product for use in commercial kitchens. Findings will be confirmed later in February after further investigations are completed. Sales of the implicated product have been suspended in the country, according to officials.

Sweden – Sweden reports increase in Yersinia cases

The Swedish Public Health Agency, or Folkhälsomyndigheten reports an increase in cases of Yersinia infections since the second week of January, according to a February 3 release.

They say more than twice as many people have contracted Yersinia infection as during the same period in the immediately preceding years.

An increased number of patients has mainly been observed in the regions of Stockholm, Västra Götaland and Halland, which together account for 33 of the total of 48 disease cases reported since 11 January.

Research – From Cheese-Making to Consumption: Exploring the Microbial Safety of Cheeses through Predictive Microbiology Models

Cheeses are traditional products widely consumed throughout the world that have been frequently implicated in foodborne outbreaks. Predictive microbiology models are relevant tools to estimate microbial behavior in these products. The objective of this study was to conduct a review on the available modeling approaches developed in cheeses, and to identify the main microbial targets of concern and the factors affecting microbial behavior in these products. Listeria monocytogenes has been identified as the main hazard evaluated in modelling studies. The pH, aw, lactic acid concentration and temperature have been the main factors contemplated as independent variables in models. Other aspects such as the use of raw or pasteurized milk, starter cultures, and factors inherent to the contaminating pathogen have also been evaluated. In general, depending on the production process, storage conditions, and physicochemical characteristics, microorganisms can grow or die-off in cheeses. The classical two-step modeling has been the most common approach performed to develop predictive models. Other modeling approaches, including microbial interaction, growth boundary, response surface methodology, and neural networks, have also been performed. Validated models have been integrated into user-friendly software tools to be used to obtain estimates of microbial behavior in a quick and easy manner. Future studies should investigate the fate of other target bacterial pathogens, such as spore-forming bacteria, and the dynamic character of the production process of cheeses, among other aspects. USA – Multi-state Salmonella outbreak of unknown origin now up to 60 patients

Traceback efforts continue in a Salmonella Miami outbreak, but few details are available. Federal investigators report that the patient count has increased to 60. The source has not yet been determined.

In its original outbreak notification on Jan. 13 the Food and Drug Administration’s weekly CORE investigation table listed 48 patients. The FDA has not revealed what states are involved in the outbreak. The Centers for Disease Control and Prevention have not released information about the outbreak.

The CORE investigation table indicates that traceback is underway, but it does not provide any other information about what food or foods are part of that effort. The table refers the public to general information pages on Salmonella and the FDA investigation process.

When the outbreak was added to the weekly CORE update an FDA spokesperson told Food Safety News the agency just doesn’t have enough information on the traceback efforts yet.
