Food contaminated with Clostridium botulinum toxin may not look or smell spoiled but can still make you sick.

Symptoms in adults can include facial paralysis or loss of facial expression, unreactive or fixed pupils, difficulty swallowing, drooping eyelids, blurred or double vision, difficulty speaking or including slurred speech, and a change in sound of voice, including hoarseness.

Symptoms of foodborne botulism in children can include difficulty swallowing, slurred speech, generalized weakness and paralysis. In all cases, botulism does not cause a fever. In severe cases of illness, people may die.

This recall was triggered by Canadian Food Inspection Agency (CFIA) test results. The CFIA is conducting a food safety investigation, which may lead to the recall of other products. If other high-risk products are recalled, the CFIA will notify the public through updated Food Recall Warnings.

The CFIA is verifying that industry is removing the recalled products from the marketplace.

Recommendation

FDA, along with CDC and state and local partners, are investigating an outbreak of 14 illnesses caused by E. coli O103 in Iowa, Illinois, Missouri, Texas and Utah. CDC is reporting that the epidemiologic evidence indicates that clover sprouts from Jimmy John’s restaurants are a likely source of this outbreak.

The CDC is recommending that consumers who have recently eaten clover sprouts on a sandwich from Jimmy John’s restaurants and who are experiencing symptoms of E. coli infection, such as severe stomach cramps, diarrhea, fever, nausea, and/or vomiting, should contact their doctor and report their illness to their local health department. If consumers have a leftover sandwich with clover sprouts from Jimmy John’s, do not eat it and throw it away.

Generally, it is recommended that children, the elderly, pregnant women, and persons with weakened immune systems should avoid eating raw sprouts of any kind.

Jimmy John’s LLC reported that all of its restaurants stopped serving clover sprouts on February 24, 2020. At this time, clover sprouts should not be available at Jimmy John’s.

FDA has activated a team to coordinate this outbreak investigation. This team is currently collecting records and initiating a traceback investigation to determine the source of the clover sprouts. Authorities are investigating whether implicated sprouts have been distributed elsewhere and will continue monitoring for additional illnesses.

Background

On February 21, 2020, the FDA issued a warning letter to Jimmy John’s Franchise LLC for engaging in a pattern of receiving and offering for sale adulterated fresh produce, specifically clover sprouts and cucumbers. On February 24, 2020, Jimmy John’s reported to FDA that they had removed sprouts from their restaurants.

The warning letter lays out evidence from five outbreaks, including recent outbreaks in the state of Iowa during November and December 2019, of human infections with Escherichia coli O103, a Shiga toxin producing Escherichia coli (STEC) linked to sprouts.

These 14 illnesses are new and are not associated with the outbreaks mentioned in the Jimmy Johns warning letter. FDA will continue its investigation and provide updates as more information becomes available.

Canada – Unomundo brand tuna loins and steaks recalled due to histamine

High levels of histamine in fish can cause an allergic-type reaction known as scombroid poisoning. Symptoms can include burning throat, diarrhea, dizziness, facial swelling, headache, vomiting and peppery taste in mouth.

USA – E. coli O103 Jimmy John’s Clover Sprouts Outbreak Sickens 22 in Iowa

An E. coli O103 Jimmy John’s clover sprouts outbreak has sickened at least 22 people in Iowa, according to a warning letter the FDA sent to Jimmy John’s and Sprouts Unlimited on February 21, 2020. This outbreak was briefly mentioned by the FDA in a recall notice on December 18, 2019, but there were very few details about the outbreak at that time.

India – More than 100 people fall sick after consuming food at a feast

Dehradun: More than 100 people fell sick after consuming food at a feast in Basantipur area of Udham Singh Nagar district. Majority of the sick locals were admitted to a Gadarpur-based community health centre on Friday. Food poisoning is suspected.

USA – Possible Poisoning Outbreak at Jimmy John’s on Laclede in St. Louis

A possible food poisoning outbreak at a Jimmy John’s restaurant at 3822 Laclede Avenue in St. Louis, Missouri may have sickened “multiple” students who attend St. Louis University. The University News, the newspaper for that campus, published this information on February 20, 2020.

The article states that multiple students got sick “shortly after eating at the sandwich shop, some even being hospitalized.” It seems that these illnesses happened in mid to late January 2020. The students attended a campus retreat for an SLU organization and that retreat was catered by Jimmy John’s.

More than a dozen students were sickened after attending that retreat and eating the food provided by Jimmy John’s. They suffered from weakness and vomiting. The paper claims that the restaurant was shut down about a week after the illnesses occurred. There is no information about this closure on the City of St. Louis Health Department web site.

Canada – Silani Mozzarella Ball Recalled in Canada for Generic E. coli

Silani Sweet Cheese Ltd. is recalling Silani Mozzarella Ball for possible generic E. coli contamination. That means this E. coli does not necessarily cause human illness, but it is an indicator of contamination with fecal matter. Generic E. coli is found in the intestines of animals.

The recalled product was sold nationally in Canada at the retail level. This is a Class 2 classification recall, which means “in which the use of, or exposure to, a violative product may cause temporary adverse health consequences or where the probability of serious adverse health consequences is remote.”

The recalled product is Silani Mozzarella Ball, sold in 260 gram packages. The UPC number on the product is 0 65052 51369 4, and the code on the product is Best Before: 2021.01.08.

India – Telangana: 50 KGBV students fall ill due to food poisoning

About 50 students of Kasturba Gandhi Balika Vidyalaya (KGBV) after having dinner late on Thursday. Sources said that before going to bed, many students complained of stomach ache and vomiting sensation.

 

UK – FSA research suggests new higher estimates for the role of food in UK illness

A scientific review by the Food Standards Agency (FSA) estimating that around 2.4 million cases of foodborne illness occur every year in the UK has been published. This is up from the 2009 estimate of approximately one million.

The FSA is also publishing a ground-breaking five-year study into the extent of norovirus in food carried out by a consortium of UK scientists, and a further FSA paper which reviews and updates the assessment developed during that project.

These new figures do not indicate an increase in total illness, or any new risk to public health, but rather provide a better estimation of the proportion of infectious intestinal disease that is due to food. The overall estimate for this type of illness, from all sources, remains the same, at around 18 million cases each year in the UK.

These new studies and their accompanying models reveal:

  • an estimated 380,000 cases of norovirus linked to food occur in the UK per year
  • a breakdown of the roles of the main transmission pathways in food suggest eating out accounts for an estimated 37% of all foodborne norovirus cases, takeaways at 26%, open-headed lettuce on retail sale at 30%, raspberries on retail sale at 4%, and oysters on retail sale at 3%
  • the revised foodborne norovirus estimate, combined with better analysis of how many illnesses of unknown cause are also likely to be caused by food, suggest around 2.4 million estimated UK cases of foodborne illness occur each year

Professor Guy Poppy, Chief Scientific Adviser to the Food Standards Agency, said:

‘This work gives us a much better idea of the role of food in the spread of all infectious intestinal disease in the UK. However, this does not mean more people are getting unwell, only that we estimate food is responsible for more existing cases than previously thought.

‘Most of this increase is due to innovative new research into foodborne norovirus. As part of this, sampling surveys focused on the five most common food-related transmission routes. Although the percentages may appear striking, the risk to consumers remains very low for most of these pathways. For example, on average, an individual would only end up with norovirus once in every 15,000 portions of open-headed lettuce – that would take around 40 years. Oysters pose the highest risk per serving, with illness likely on average once in around 160 servings.

‘We are not changing our advice to consumers and businesses. Instead this research reinforces the need for the highest standards of good personal and food hygiene practices in catering establishments and at home to avoid infection.’

Professor Sarah J. O’Brien, lead NoVAS researcher, added:

‘Estimating the contribution of norovirus to the burden of UK foodborne disease has been particularly difficult up until now. This is largely due to people not attending doctors’ surgeries when they are unwell with symptoms of the winter vomiting bug. And whilst helpful in preventing the spread of the virus and alleviating the strain on healthcare settings, it does mean that crucial information about the virus cannot be collected. This is why the development of the first risk assessment of this type for the UK is particularly significant.’

The FSA can now use this new and improved understanding of the significance of foodborne disease to inform future efforts to control and reduce the risk of infection posed to the public from food by all pathogens.
