Human infective noroviruses (NoVs) are a worldwide leading cause of foodborne illness and are frequently spread via infected food handlers preparing and manipulating food products such as deli sandwiches. The objective of the current study was to determine the efficiencies whereby NoV could be transferred between surfaces associated with the preparation of manually prepared foods such as deli sandwiches. Nonfood surfaces included gloves and stainless steel discs, and boiled ham, lettuce, and a sandwich bun were the ingredients of the deli sandwich. Both NoV GII.4 and the murine NoV 1 (MNV-1, a cultivable human NoV surrogate) were included in the presented study. Transfer of NoV GII.4 and MNV-1 between surfaces was performed by pressing an inoculated donor surface against an acceptor surface. To evaluate the effect of subsequent contact, donor surfaces were pressed a second time to an identical acceptor surface. Subsequently, NoV GII.4 and MNV-1 were detected using real-time reverse transcription PCR assays and plaque assays, respectively. Transfer of both viruses from gloves to stainless steel was inefficient, and virus transfer from food products to stainless steel occurred with more variability for NoV GII.4 than for MNV-1. Virus transfer from the stainless steel discs to the gloves was substantially more efficient than from the gloves to the stainless steel. NoV GII.4 and MNV-1 transfer from food products to the gloves occurred with varying efficiencies, although this variation was more evident for NoV GII.4. The MNV-1 inoculum was significantly less efficiently transferred to the acceptor surface at the second contact, which was not the case for NoV GII.4. The obtained transfer efficiency data may provide insights into the transfer of NoV during preparation of foods and can be included in risk assessment models describing the transmission of NoVs in this context.

Human noroviruses (HuNoVs) are the most common cause of acute viral gastroenteritis worldwide and are a leading cause of foodborne disease. Their environmental persistence and purported resistance to disinfection undoubtedly contribute to their success as foodborne disease agents. The purpose of this study was to compare the efficacy of three commonly used disinfectant active ingredients against representative HuNoV strains and cultivable surrogates. Ethanol (50, 70, and 90%), sodium hypochlorite (5, 75, 250, 500, and 1,000 ppm), and a quaternary ammonium compound blend (at 0.1×, 1.0×, and 10× concentrations) were evaluated against two norovirus (NoV) genogroup II strains (GII.2 and GII.4) and two surrogates (feline calicivirus [FCV] and murine norovirus [MNV-1]). Virucidal suspension assays (30-s exposure) were conducted in accordance with ASTM International standard E-1052. Virus inactivation was quantified using reverse transcription quantitative PCR targeting the ORFI-ORFII junction (HuNoV), the RNA polymerase region (MNV-1), or the ORFI region (FCV); infectivity assays were also performed for MNV-1 and FCV. The two HuNoV strains and FCV were relatively resistant to ethanol (<0.5 log inactivation) irrespective of concentration, but MNV-1 was much more susceptible (log inactivation, ∼2.0 log at higher ethanol concentrations). Both HuNoV strains were more resistant to hypochlorite than were either of the animal surrogates, with the human strains requiring ≥500 ppm of hypochlorite to achieve statistically significant reduction (≥3.0 log) in virus concentration. All four viruses were resistant to inactivation (<0.5-log reduction) using the quaternary ammonium compound formulation at all concentrations tested. This study is novel in that it clearly demonstrates the relative ineffectiveness of common active disinfectant ingredients against HuNoV and highlights the fact that the cultivable surrogates do not always mimic HuNoV strains.

Some of the causes include viruses, bacteria, bacterial toxins, parasites, particular chemicals and some drugs. The main complication of gastroenteritis is dehydration, but this can be prevented if the fluid lost in vomit and diarrhoea is replaced.

This is the third annual report that measures our success in receiving early warning on problems with food and feed. The Reportable Food Registry (RFR) has already proven itself an invaluable tool to help prevent contaminated food from reaching the public.

By providing early warning about potential public-health risks from reportable foods, the Registry increases the speed with which the FDA, its state- and local-level partners, and industry can remove hazards from the marketplace.

The RFR data also is providing valuable data to help meet requirements under the Food Safety Modernization Act. For example, we can use the data to identify hazards associated with products for which we have not previously made such an association and thus identify foods for which preventive controls may be needed. The data are also being used to help target inspections, plan work, identify and prioritize risks and develop guidance for industry. The FDA will continue working closely with the food and feed industries to enhance this important and beneficial tool.

Human norovirus is the leading cause of epidemic gastroenteritis, especially in semiclosed settings such as daycares, nursing homes, hospitals, schools and on cruise ships. Outbreaks are often accompanied by contamination of environmental surfaces and commonly handled items. Surface disinfection of norovirus surrogates, feline calicivirus and murine norovirus, by 20 parts per million atmospheric ozone in a chamber maintaining 80% relative humidity was investigated. After treatment, neither virus could be detected on glass surfaces, but >5 log infectious virus was recovered from untreated controls. Ozone chambers used to decontaminate small, hand-contact items could be an important tool for controlling norovirus outbreaks

Norovirus (NoV) infections are the leading cause of foodborne illness in the United States. Effective disinfection is important for controlling outbreaks caused by this highly infectious virus but can be difficult to achieve because NoV is very resistant to many common disinfection protocols. The inability of human NoV to replicate in tissue culture complicates NoV research, generally necessitating genome copy quantification, the use of surrogate viruses, or the use of other substitutes such as virus-like particles. To date, comprehensive comparisons among NoV surrogates and between surrogates and human NoV are missing, and it is not clear how best to extrapolate information from surrogate data. We conducted a systematic review and meta-analysis of comparisons of NoV surrogates with regard to their susceptibility to disinfection on hard surfaces or in suspension. Restricting our analysis to those studies in which two or more virus surrogates were compared allowed us to improve the signal-to-noise ratio in our analysis, similar to the epidemiological concept of matching. Using meta-analysis methods, our results indicate that hepatitis A virus, murine norovirus 1, and phage MS2 are significantly more resistant to disinfection than is feline calicivirus, but average differences in viral titer reduction appeared to be modest, 1.5 log PFU or less in all cases. None of the studies that compared surrogates and human NoV met our inclusion criteria, precluding a direct comparison between human NoV and NoV surrogates in this study. For all surrogates with sufficient data available to permit subgroup analyses, we detected strong evidence that the type of disinfectant impacted the relative susceptibility of the surrogates. Therefore, extrapolation of results between surrogates or from surrogates to human NoV must consider the type of disinfectant studied.

The occurrence and relevance of food incidents notified through the Rapid Alert System for Food and Feed, the findings of audits to third countries carried out by the Food and Veterinary Office, as well as the quarterly reports on consignments of feed and food of non- animal origin submitted by Member States to the Commission in accordance with Article 15 of Regulation (EC) No 669/2009 indicate that the list should be amended.

In particular, for consignments of dried vine fruit from Afghanistan, watermelons from Brazil, strawberries from China, peas and beans from Kenya, mint from Morocco, watermelon seeds and derived products from Sierra Leone and certain herbs, spices and vegetables from Vietnam, the relevant sources of information indicate the emergence of new risks and/or a degree of non- compliance with the relevant safety requirements, thereby warranting the introduction of an increased level of official controls. Entries concerning those consignments should be therefore included in the list.

USA – Norovirus Update Mulvaney Restaurant

The outbreak of norovirus at Mulvaney’s restaurant in Sacramento we told you about in March has been confirmed and updated. Laura McCasland, Communication & Media Officer of the Sacramento County Department of Health & Human Services told us that 138 people were sickened in this outbreak.

The ill persons ate at the restaurant between Friday, February 22 and Tuesday, February 26, 2013. During the week of February 17 to February 24, 2013, at least six food service workers reported that they were ill. People started gettting sick between Friday, February 22, 2013 and Thursday, February 28, 2013. The cause of the illness was norovirus. The restaurant was inspected February 13, 2013 and passed.

Public health officials suspect that the foods that are associated with the risk of developing illness include Turkish coffee pudding, pickled beet salad, grilled eggplant, ham, lamb, and Yukon potatoes. The age range for all of the events was 18 to 80 years old. Among all of the ill patrons who ate at Mulvaney’s restaurant, 52.4% were female and 47.6% were male.

New research presented last week at the American Society for Microbiology’s 2013 general meeting showed that copper can destroy norovirus. The research was conducted at the University of Southampton. That virus causes more than 20 million illnesses in the United States every year, according to the Centers for Disease Control and Prevention, and norovirus causes 267 million infections worldwide every year.

Norovirus is extremely contagious and often affects people in contained situations, such as cruise ships and retirement homes. It is spread through person-to-person contact, contact with contaminated objects, and through food and drink.

Scientists discovered that norovirus is quickly destroyed on contact with copper.

Two Northumbria University students who became ill after their graduation ball have tested positive for vomiting bug, norovirus.

Around 30 people complained of sickness after the event at the Assembly Rooms on May 10.

Experts from the North East Public Health England Centre are working with environmental health colleagues at Newcastle City Council to investigate the outbreak of diarrhoea and vomiting.

A spokesperson for Public Health England said: “Approximately 30 students who attended an event in Newcastle on 10 May have reported symptoms of illness.

“Two samples taken from students who were ill have tested positive for norovirus (also known as the winter vomiting bug) which is the most common cause of gastro-enteritis in England and Wales.”
