Research – Natural Gut Flora Protection Against E.coli O157

A cocktail of non-pathogenic bacteria naturally occurring in the digestive tract of healthy humans can protect against a potentially lethal E. coli infection in animal models according to research presented today at the 113th General Meeting of the American Society for Microbiology. The research, conducted by scientists at the University of Michigan Medical School in Ann Arbor, could have important implications for the prevention or even treatment of this disease.

Food microbiology laboratories continue to submit false negative results and false positive results on a routine basis. A retrospective study of nearly 40,000 proficiency test results over the past 14 years, presented today at the 113th General Meeting of the American Society for Microbiology, examined the ability of food laboratories to detect or rule out the presence of Escherichia coli O157:H7, Salmonella, Listeria monocytogenes, and Campylobacter.

The study found that, on average, food laboratories report false negatives of 9.1% for Campylobacter, a bacterial foodborne illness that may cause bloody diarrhea, cramping and fever, and 4.9% for Salmonella, a bacteria that may cause diarrhea, fever and abdominal cramps sometimes leading to hospitalization or death. The false positive rate, on average, is 3.9% for Salmonella, and 2.5% for both E. coli and L. monocytogenes.

USA – E.coli O157:H7 Outbreak

At least seven people in Stephens County, Ga., have fallen ill with E. coli in the last several weeks, leading health officials in the area to begin investigating the source of the outbreak, local radio station WNEG reports.

Two of the patients were hospitalized and are now recovering. Most of the seven sickened have been confirmed to have E. coli O157:H7.

Research – Study Pinpoints Pathogens Causing Biggest Diarrhea Burden in Kids

While many pathogens cause serious diarrhea in children in developing countries, four stand out as culprits in most of the infections, a finding that could drive new prevention strategies, an international research team reported today.

The group spent 3 years in seven developing countries, collecting health information, lab samples, and outcome data on 3,439 youngsters who had moderate-to-severe diarrhea. They published their findings today in an early online release from The Lancet.

On a global scale each year, diarrheal diseases kill about 800,000 children under age 5, most of them in sub-Saharan Africa and southern Asia, according to the report.

Though interventions such as rotavirus vaccines are starting to have an impact on deaths and disease incidence, scientific data to guide other strategies to curb the diseases are scarce, especially in regions where child deaths are the highest, the group wrote.

To fill the gap, the team focused their analysis on kids under age 5 who did and didn’t have moderate-to-severe diarrhea in selected cities in Bangladesh, Gambia, India, Kenya, Mali, Mozambique, and Pakistan. The study included 13,129 matched controls.

The researchers collected clinical and epidemiologic information, took anthropometric measurements, and took fecal samples to identify pathogens. For each child they made a follow-up home visit 60 days later to assess health status, clinical outcome, and growth measures.

Four pathogens were the most common causes of diarrhea across all sites: rotavirus, Cryptosporidium, enterotoxigenic Escherichia coli (ETEC) producing heat-stable toxin (ST), and Shigella, according to the study.

 

 

USA – Texas E.coli O157:H7 Outbreak

Health investigators in Texas’ Brazos County are looking into eight possible infections of E. coli O157:H7 they suspect may be linked to food.

The Brazos County Health Department has confirmed five of the illnesses, and investigators believe another three patients are infected with the same strain of bacteria.

Two related children under the age of five have been hospitalized at Texas Children’s Hospital in Houston for the past week, The Eagle reports. The other six cases are adults and were not hospitalized.

Investigators are still searching for a possible outbreak source, saying that the patients did not eat at the same restaurant.

USA – Raw Milk – E.coli O157 Outbreak

Health officials in Wisconsin suspect three patients sickened by the same strain of E. coli O157:H7 contracted their illnesses after consuming raw milk, Wisconsin Department of Agriculture spokeswoman Raechelle Cline told Food Safety News Thursday.

All three patients reside in Manitowoc County, and they include a three year-old child and his or her mother. Officials are currently testing samples of milk from the suspected dairy and will not identify it unless they prove a connection.

“Raw milk was the most likely commonality we’ve been able to identify,” Cline said. The illnesses occurred in March, and the officials are unaware of any additional cases.

Health institutions such as the U.S. Centers for Disease Control and Prevention advise against drinking raw milk, as it has not been pasteurized to eliminate potentially harmful pathogens.

EFSA’s scientific experts say that it is currently not possible to identify which VTEC bacteria strains have the potential to cause human diseases. In order to help risk managers to identify human health risks, EFSA has proposed a scheme to categorise VTEC strains according to their potential to cause disease. This work has been carried out in response to a request of the Austrian Federal Ministry of Health.

VTEC (verocytotoxin-producing Escherichia coli) is a group of pathogenic E. coli bacteria that can cause bloody diarrhoea and haemolytic uremic syndrome in humans, a serious condition that can lead to kidney failure and be fatal [1].

EFSA’s Panel on Biological Hazards (BIOHAZ) has evaluated data from the EU on different VTEC strains with respect to their reported frequency, severity of human disease caused by the strains, and association with outbreaks. The Panel concluded that it is currently not possible to fully predict the potential of a VTEC strain found in food to cause human disease.

However the Panel has provided guidance to assist public health authorities in assessing risks related to VTEC strains. They proposed a scheme that considers the detection of specific genes in VTEC strains from humans, food and animals. EFSA will regularly review this scheme to improve future risk assessments.

Due to under-reporting of human cases and the unavailability of complete information, EFSA’s scientific experts also recommended that all Member States collect comprehensive data on VTEC strains when these are detected. Accurate reporting will help in predicting the factors responsible for the severity of human infections and outbreaks.

To carry out this evaluation, the Panel used data from the European Surveillance System (TESSy data) provided by the European Centre for Disease Prevention and Control and from the EU Summary Report on Trends and Sources of Zoonoses, Zoonotic Agents and Food-borne Outbreaks in 2011published today as well as data from relevant scientific literature.

The European Food Safety Authority and the European Centre for Disease Prevention and Control analysed the information submitted by 27 European Union Member States on the occurrence of zoonoses and food-borne outbreaks in 2011. Campylobacteriosis was the most commonly reported zoonosis with 220,209 confirmed human cases. The occurrence of Campylobacter continued to be high in broiler meat at EU level.

The decreasing trend in confirmed salmonellosis cases in humans continued with a total of 95,548 cases in 2011. Most Member States met their Salmonella reduction targets for poultry, and Salmonella is declining in these populations. In foodstuffs, Salmonella was most often detected in meat and products thereof.

The number of confirmed human listeriosis cases decreased to 1,476. Listeria was seldom detected above the legal safety limit from ready-to-eat foods.

A total of 9,485 confirmed verotoxigenic Escherichia coli (VTEC) infections were reported. This represents an increase of 159.4 % compared with 2010 as a result of the large STEC/VTEC outbreak that occurred in 2011 in the EU, primarily in Germany. VTEC was also reported from food and animals.

The number of human yersiniosis cases increased to 7,017 cases. Yersinia enterocolitica was isolated also from pig meat and pigs; 132 cases of Mycobacterium bovis and 330 cases of brucellosis in humans were also reported. The prevalence of bovine tuberculosis in cattle increased, and the prevalence of brucellosis decreased in cattle and sheep and goat populations.

Trichinellosis and echinococcosis caused 268 and 781 human cases, respectively and these parasites were mainly detected in wildlife. The numbers of alveolar and of cystic echinococcosis respectively increased and decreased in the last five years. One imported human case of rabies was reported. The number of rabies cases in animals continued to decrease.

Most of the 5,648 reported food-borne outbreaks were caused by Salmonella, bacterial toxins, Campylobacter and viruses, and the main food sources were eggs, mixed foods and fish and fishery products.
