UK -UKHSA issue warning as infections rise in East of England

Families visiting farms and petting zoos this summer are being urged to protect themselves following a rise in infections.

The UK Health Security Agency (UKHSA) has issued a warning about gastrointestinal infections such as salmonella, cryptosporidium and e.coli.

They can all be passed from farm animals to humans, causing illness.

There have already been 92 cases of cryptosporidium in the east of England so far this year, 14 more than in all of 2022.

So the UKHSA is urging anyone visiting a farm to remember the importance of thoroughly and frequently washing hands to avoid getting the bugs, which can make you seriously ill.

The Animal and Plant Health Agency (APHA) was involved in another two E. coli outbreaks in the United Kingdom in the final quarter of 2022.

In the first, APHA helped Public Health Wales investigate two human cases of Shiga toxin-producing E. coli (STEC) O145 linked to a private collection of animals on a smallholding. Multiple species were onsite including cattle, goats, deer, and pigs. Some food consumed by people was grown in the garden where cattle manure was used. Thirty environmental fecal samples were collected but a match to the outbreak strain was not found.

In the second, APHA visited an open farm at the request of an incident management team following an outbreak of E. coli O157 in people. The outbreak strain was detected in one environmental sample from a pig enclosure. The incident is ongoing so it is unclear how many people are sick. The advice was provided to reduce the risk to the public by making improvements to the supervision of animal contact, enhancing handwashing facilities, and improving some animal exhibits.

In all of 2022, APHA was part of five E. coli investigations. The agency helped the UK Health Security Agency (UKHSA) investigate E. coli O103, O145, and O26 outbreaks between July and September.

The E. coli O26 outbreak also involved cryptosporidium. There were 11 cases of cryptosporidium and two people had confirmed E. coli O26. Cryptosporidium patients had visited an open farm attraction during the incubation period of illness. The E. coli patients had links to the same premises.

The E. coli O103 outbreak with 11 cases was associated with soft, raw milk cheese from a dairy farm in the East of England. Pasteurization was put in place for the production of the soft cheese, HACCP processes were reviewed, and enhanced control measures were taken.

The E. coli O145 outbreak with 10 patients was traced to the consumption of milk products from a dairy farm in North West England, with illness onset from mid-July. Investigations identified an issue with pasteurization and problems with the cleaning and storage of milk crates.

Research – Avoiding a global chocolate disaster – how tracing and recalls avoided a worldwide Salmonella outbreak

Largest ever recall of chocolate products in global history, just before Easter 2022, prevented thousands of extra cases; a total of 455 cases of Salmonella Typhimurium found in 17 countries; UK had most cases with 128.

Like any other manufactured food product, chocolate can be contaminated if key ingredients or processes break down. In a presentation in a pre-ECCMID day for this year’s European Congress of Clinical Microbiology & Infectious Diseases (ECCMID 2023, Copenhagen, 15-19 April), Dr Johanna Takkinen, Principal Expert for Food- and Waterborne Diseases at the European Centres for Disease Control and Prevention (ECDC), Stockholm, Sweden, will discuss the drama as the story unfolded, and the lessons learned from an outbreak of Salmonella Typhimurium in Kinder Chocolate Eggs traced to a Belgian chocolate factory.

”If not for clear and co-ordinated action across Europe and beyond, there may have been many thousands more children falling ill, and potentially many deaths,” says Dr Takkinen.

Authorities in the UK (the UK Health Security Agency [UKHSA]) first raised the alarm in ECDC-hosted alert platform EpiPulse on 17 February 2022, reporting a cluster of 18 children reported ill with monophasic Salmonella Typhimurium infections since January 2022. Of these, seven were hospitalised and five of the seven had bloody diarrhoea, a serious symptom. “Preliminary interviews of first cases indicated Kinder chocolate products as a possible vehicle of infection. Several countries then began reporting an increasing number of infections with strains the same as the UK outbreak,” explains Dr Takkinen. By 18 February, France had reported its first 2 cases, and by 18 March 59 cases were reported in five countries.

Late in March 2022, ECDC coordinated a teleconference with affected countries when four non-human monophasic S. Typhimurium isolates, genetically close to the human isolates, were identified in a public database. Within a week, these isolates were confirmed originating from one particular Belgian chocolate factory. Prior to this, identifying which factory or factories were involved was difficult since there are four factories within the European Union that produce Kinder chocolate in large quantities. This new microbiological evidence allowed the various agencies to focus their investigations on one factory.

Meanwhile, the Food Standards Agency (FSA) in the UK* and the Food Safety Authority (FSA) in Ireland and the UK FSA decided to recall, on April 2**, certain Kinder Chocolate products (including Kinder Surprise Eggs).  On April 8 authorities, now confident the factory was identified, ordered that chocolate factory (Ferrero) closed, and two days later had issued a global recall of products from the factory. The alert reached 130 countries, and in addition to the 401 cases* identified in the EU and UK combined (the UK had the most cases, with 128), further cases were identified in Switzerland (49) and Canada (4) and the USA (1) – giving a global total of 455 cases in 17 countries.  The ECDC and EFSA also published Rapid Outbreak Assessments to keep the public updated.

UK – Three dead in UK fish Listeria outbreak

Three people have died in an ongoing Listeria outbreak linked to fish in the United Kingdom.

The UK Health Security Agency, Food Standards Agency (FSA), Public Health Scotland and Food Standards Scotland (FSS) are investigating the Listeria monocytogenes outbreak that was first made public in April.

Twelve patients across the UK have been identified, including three in Scotland, with onset dates of illness between October 2020 to March 2022.

Ten sick people are over the age of 65, and one is a pregnant woman. Three of those affected who are older than 65 have died.

Six people have fallen ill since January 2022. The majority of patients reported eating smoked fish.

The FSA and FSS are doing food chain and microbiological investigations including whole genome sequencing (WGS) but the cause is yet to be confirmed.

A survey of Salmonella, Escherichia coli (E. coli) and antimicrobial resistance in frozen, part-cooked, breaded or battered poultry products on retail sale in the United Kingdom

In this study we estimated how frequently Salmonella spp. were present in frozen, breaded or battered chicken products, intended to be cooked before consumption, on retail sale in the UK between April and July 2021.

Frozen, breaded, ready-to-cook chicken products have been implicated in outbreaks of salmonellosis. Some of these outbreaks can be large. For example, one outbreak of Salmonella Enteritidis involved 193 people in nine countries between 2018 and 2020, of which 122 cases were in the UK. These ready-to-cook products have a browned, cooked external appearance, which may be perceived as ready-to-eat, leading to mishandling or undercooking by consumers. Continuing concerns about these products led FSA to initiate a short-term (four month), cross-sectional surveillance study undertaken in 2021 to determine the prevalence of Salmonella spp., Escherichia coli and antimicrobial resistance (AMR) in frozen, breaded or battered chicken products on retail sale in the UK.

This study sought to obtain data on AMR levels in Salmonella and E. coli in these products, in line with a number of other FSA instigated studies of the incidence and nature of AMR in the UK food chain, for example, the systematic review (2016).

Between the beginning of April and the end of July 2021, 310 samples of frozen, breaded or battered chicken products containing either raw or partly cooked chicken, were collected using representative sampling of retailers in England, Wales, Scotland and Northern Ireland based on market share data. Samples included domestically produced and imported chicken products and were tested for E. coli (including extended-spectrum beta-lactamase (ESBL)-producing, colistin-resistant and carbapenem-resistant E. coli) and Salmonella spp. One isolate of each bacterial type from each contaminated sample was randomly selected for additional AMR testing to determine the minimum inhibitory concentration (MIC) for a range of antimicrobials. More detailed analysis based on Whole Genome Sequencing (WGS) data was used to further characterise Salmonella spp. isolates and allow the identification of potential links with human isolates.

Salmonella spp. were detected in 5 (1.6%) of the 310 samples and identified as Salmonella Infantis (in three samples) and S. Java (in two samples). One of the S. Infantis isolates fell into the same genetic cluster as S. Infantis isolates from three recent human cases of infection; the second fell into another cluster containing two recent cases of infection. Countries of origin recorded on the packaging of the five Salmonella contaminated samples were Hungary (n=1), Ireland (n=2) and the UK (n=2). One S. Infantis isolate was multi-drug resistant (i.e. resistant to three different classes of antimicrobials), while the other Salmonella isolates were each resistant to at least one of the classes of antimicrobials tested. E. coli was detected in 113 samples (36.4%), with counts ranging from ❤ to >1100 MPN (Most Probable Number)/g. Almost half of the E. coli isolates (44.5%) were susceptible to all antimicrobials tested. Multi-drug resistance was detected in 20.0% of E. coli isolates. E. coli isolates demonstrating the ESBL (but not AmpC) phenotype were detected in 15 of the 310 samples (4.8%) and the AmpC phenotype alone was detected in two of the 310 samples (0.6%) of chicken samples. Polymerase Chain Reaction (PCR) testing showed that five of the 15 (33.3%) ESBL-producing E. coli carried blaCTX-M genes (CTX-M-1, CTX-M-55 or CTX-M-15), which confer resistance to third generation cephalosporin antimicrobials. One E. coli isolate demonstrated resistance to colistin and was found to possess the mcr-1 gene.

The five Salmonella-positive samples recovered from this study, and 20 similar Salmonella-positive samples from a previous UKHSA (2020/2021) study (which had been stored frozen), were subjected to the cooking procedures described on the sample product packaging for fan assisted ovens. No Salmonella were detected in any of these 25 samples after cooking.

The current survey provides evidence of the presence of Salmonella in frozen, breaded and battered chicken products in the UK food chain, although at a considerably lower incidence than reported in an earlier (2020/2021) study carried out by PHE/UKHSA as part of an outbreak investigation where Salmonella prevalence was found to be 8.8%.

The current survey also provides data on the prevalence of specified AMR bacteria found in the tested chicken products on retail sale in the UK. It will contribute to monitoring trends in AMR prevalence over time within the UK, support comparisons with data from other countries, and provide a baseline against which to monitor the impact of future interventions. While AMR activity was observed in some of the E. coli and Salmonella spp. examined in this study, the risk of acquiring AMR bacteria from consumption of these processed chicken products is low if the products are cooked thoroughly and handled hygienically.
