Research – New study sheds light on how Salmonella spreads in the body

Findings of Cambridge scientists, published today in the journal PLoS Pathogens, show a new mechanism used by bacteria to spread in the body with the potential to identify targets to prevent the dissemination of the infection process.

Salmonella enterica is a major threat to public health, causing systemic diseases (typhoid and paratyphoid fever), gastroenteritis and non-typhoidal septicaemia (NTS) in humans and in many animal species worldwide. In the natural infection, salmonellae are typically acquired from the environment by oral ingestion of contaminated water or food or by contact with a carrier.  Current vaccines and treatments for S. enterica infections are not sufficiently effective, and there is a need to develop new therapeutic strategies.

Dr Andrew Grant, lead author of the study from the University of Cambridge, said: “A key unanswered question in infectious diseases is how pathogens such as Salmonella grow at the single-cell level and spread in the body.  This gap in our knowledge is hampering our ability to target therapy and vaccines with accuracy.”

Research – Safer Spinach Combined Microbiological Control

University of Illinois scientists have found a way to boost current industry capabilities when it comes to reducing the number of E. coli 0157:H7 cells that may live undetected on spinach leaves.

“By combining continuous ultrasound treatment with chlorine washing, we can reduce the total number of foodborne pathogenic bacteria by over 99.99 percent,” said Hao Feng, a U of I professor of food science and human nutrition.

USA – CDC – Multistate Outbreak of E.coli O157:H7 – Spinach

A total of 28 persons infected with the outbreak strain of Shiga toxin-producing Escherichia coliO157:H7 (STEC O157:H7) have been reported from five states, with most cases in New York.

42% of ill persons have been hospitalized. Two ill persons have developed hemolytic uremic syndrome (HUS), a type of kidney failure, and no deaths have been reported.

The outbreak was initially in New York. More recently, more ill persons in other states have been reported, and the investigation has expanded.

UK/Germany/Netherlands – Increase in Cryptosporidium Notifications in 2012

An increase in Cryptodsporidim notifications has been observed in the UK, Netherlands and Germany since August 2012 that is likely to be real and not due to surveillance or notifcations artifacts.

The available information from investigations in the United Kingdom, the Netherlands and Germany indicates that there is not a single, common source, but rather a combination of several causes. These may include climatic drivers, such as the increased rainfall in the summer of 2012 in these countries or a widely distributed commonly consumed product. There is however no evidence for it at this stage and further investigations are ongoing.

 The overall threat for the European Union/European Economic Area (EU/EEA) is considered to be low.

 EU/EEA Member States should be alert to an increase in cases as observed in the United Kingdom, the Netherlands and Germany, particularly in relation to immunocompromised and other at-risk groups as they may present with a more severe manifestation of cryptosporidiosis.

 

UK – London – Norovirus Outbreak

A Primrose Hill school was closed today after more than 200 pupils and 15 members of staff were struck down with the contagious Norovirus.

Health inspectors ruled the winter vomiting bug was present at North Bridge House Preparatory School. The building in Gloucester Avenue was being cleansed today amid fears the virus could spread to the school’s other buildings.

In a letter sent to parents, headteacher Brodie Bibby said the school was “afflicted by a particularly unpleasant virus.”

Any children showing signs of sickness were told to be kept at home by parents, she wrote – adding that any children still well enough were invited to attend a planned performance of Swan Lake tomorrow night.

She added that “a specialist team will clean the entire school with anti-viral sprays and use air-based products.”

One parent told a local newspaper that his daughter “suddenly turned white and projectile vomited all over her bedroom” after being struck down with the illness this week.

North Bridge House, which also has a nursery school, a junior and a senior school at different sites in Camden, will reopen on Monday.

Bivalve mollusks (shellfish), such as mussels and oysters, are filter feeders; they concentrate microorganisms of human and animal origin (up to 100×) from the surrounding environment. Several recent reports have linked the incidence of human infection with hepatitis E virus (HEV) to consumption of undercooked pork, game products, and shellfish (1,2). Infectious HEV has been found in swine manure and wastewater (3); therefore, application of manure to land and subsequent runoff could contaminate coastal water, leading to contamination of shellfish and, subsequently, possible human infection. Because they are filter feeders, bivalve mollusks are biologically relevant sentinels and can indicate potential pathogens that are contaminating the environment. It is essential to ensure that this sustainable resource of coastal areas, where mussels and oysters are farmed or collected wild, is not subjected to environmental contamination that could lead to public health risks.

Risk management for bivalve mollusks, aimed at control of fecal pollution, relies heavily on the use of Escherichia coli as an indicator of fecal (sewage) contamination and is enacted under European food regulations (Regulation 854/2004, www.cefas.co.uk/media/455777/extract_reg_no_854_2004.pdf Adobe PDF file). However, although these regulations probably reduce the number of infections, especially bacterial infections, they are not viewed as adequately controlling the risk for viral infections. Specific risks are posed by the robustness of viruses in the environment and the different behavior of viruses within bivalve mollusks compared with behavior within bacterial fecal indicators.

A novel strain of E. coli bacteria caused a serious outbreak of foodborne illness focused in Germany in 2011. In all, 3,950 people were affected and 53 died, including 51 in Germany. Authorities have identified vegetable sprouts as the source.

The recent outbreaks of food poisoning due to contamination of vegetables by dangerous strains of E. coli raised the concerns and questions about E. coli transmission strategies.

E. coli is most at home in the warm, moist, nutrient-rich environment found in the gastrointestinal tract of warm-blooded animals. But to disperse from one host to another these bacteria must get out into the world. There is evidence that some E. coli can survive for several weeks outside the host, and even grow in water or soil. But it is on plant matter that E. coli colonisation has become a concern, as although most types of E. coli are harmless, the presence of pathogenic strains on fruit and vegetables presents a food safety risk.

E. coli strains have adopted slightly different transmission strategies, with some being better adapted to live on plants than others.

BBC – School in Leeds Hit by Shigella

The Health Protection Agency (HPA) said pupils and staff at Cross Lane Primary School in Elland had been affected by the Shigella bacteria, which can cause diarrhoea and vomiting.

The HPA said that nine cases had been confirmed and more than 50 people had displayed symptoms over the last three weeks.

The school will reopen on 15 October.

Japan – Ecoli O157 in Pickled Cabbage Outbreak – Six Dead

Sapporo, Japan news services report that four people, including a 4-year-old girl, have been confirmed dead of food poisoning from pickled Chinese cabbage produced by a Sapporo food company. Another two are suspected to have died due to the cabbage.

According to the city health care center, four people have died because of the E. coli O157:H7 bacteria after eating pickled Chinese cabbage produced in late July by Iwai Shokuhin. Three of the four victims were women in their 80s and 100s living in elderly care facilities in the city.
