USA – Legionnaires’ disease: Lower Washington Heights cluster rises to 14

Outbreak News Today Legionella_Plate_01

In a follow-up on the Legionnaires’ disease cluster reported in Lower Washington Heights in northern Manhattan, New York City health officials report the case count has risen to 14.

The Health Department is actively investigating these cases and is sampling and testing water from all active cooling tower systems in the area of the cluster.

This is the second Legionnaires’ disease cluster reported in Lower Washington Heights this year.

Legionnaires’ disease is a type of pneumonia that is caused by the bacteria Legionella, which grows in warm water. Symptoms resemble other types of pneumonia and can include fever, chills, muscle aches, and cough. Most cases of Legionnaires’ disease can be traced to plumbing systems where conditions are favorable for Legionella growth, such as cooling towers, whirlpool spas, hot tubs, humidifiers, hot water tanks, and evaporative condensers of large air-conditioning systems.

Individuals may be infected by breathing in water vapor containing Legionella, and the disease is not transmitted from person to person. Individuals at higher risk include those ages 50 and above, cigarette smokers, and people with chronic lung disease or compromised immune systems. People living or working in the area who are experiencing these symptoms should seek medical attention with a primary care provider or seek urgent care.

Australia – SA Health issue Listeria warning for silverside (beef)

Outbreak News Today

South Australians, particularly pregnant women, the elderly and those with weakened immune systems, are advised not to consume pre-packed sliced silverside purchased from Drakes Supermarkets following the detection of Listeria in the product.

SA Health’s Acting Executive Director of Public Health Services, Dr Chris Lease, said as yet there have been no cases of Listeria infection reported to SA Health linked to the product.

“Following detection of Listeria during routine testing, Drakes Supermarkets are working to remove any potentially affected product from supply today,” Dr Lease said.

“While we have not received notification of anyone contracting Listeria from the product, the infection can be extremely serious if it is contracted by the elderly, pregnant women or people with weakened immune systems.

South Korea – Man, 71, had hand amputated when skin started rotting 12 hours after eating sushi – Vibrio vulnificans

New England Journal of Medicine

71-year-old man presented to the emergency department with a 2-day history of fever and excruciating pain in his left hand that had developed 12 hours after eating raw seafood. He had a history of type 2 diabetes mellitus and hypertension and was undergoing hemodialysis for end-stage renal disease. At the time of presentation, hemorrhagic bullae measuring 3.5 by 4.5 cm had developed on the palm of his left hand (Panel A), and erythematous swelling with confluent tense bullae and ecchymoses had developed on the dorsum of the hand and forearm (Panel B). Surgical intervention was performed urgently, and Vibrio vulnificus was isolated from the bullae. Postoperatively, the patient received intravenous ceftazidime and ciprofloxacin. V. vulnificus can cause skin infections after wound exposure to contaminated seawater, as well as primary septicemia through the consumption of contaminated raw or undercooked seafood. Patients with immunocompromising conditions, including chronic liver disease and cancer, are at increased risk for infection and complications. Despite treatment, the skin lesions progressed to deep necrotic ulcers, and amputation of the left forearm was performed 25 days after presentation. The patient did well after the surgery and was discharged home.

Research – Commercial Frozen Mice Used by Owners to Feed Reptiles are Highly Externally Contaminated with Salmonella Enteritidis PT8

Mary Ann Liebert

Salmonella remains one of the most prevalent zoonoses worldwide. Although salmonellosis is commonly associated with the consumption of contaminated food, it has been estimated that up to 11% of Salmonella infections overall are acquired from direct or indirect contact with animals, including reptiles. In 2016, an outbreak of Salmonella Enteritidis involving multiple cases, especially children, associated with reptile contact and contaminated feeder mice was reported in the United Kingdom. The aim of this study was to investigate Salmonella external and internal contamination of stored commercial frozen feeder mice used to feed reptiles and obtained from the same supplier involved in the outbreak. In this study a total of 295 mice were tested (60 pinkies, 60 fuzzies, 60 small, 60 large, and 55 extra large). In this study, both external (integument) and internal (selected organs) contamination were evaluated. Salmonella Enteritidis PT8 and PT13 were isolated from 28.8% (n = 17) of the 59 batches tested, with the exception of the large mice category. Positive mice were mostly contaminated externally (92.3% vs. 26.9% for carcass wash and viscera, respectively). All isolates were sensitive to all 16 antimicrobials tested. The high level of external contamination of the rodent carcasses might have played a role in the human outbreak in 2016. Reptile owner management of the rodent carcasses at home could be an important source of salmonellosis outbreaks. Collaboration among public health officials, pet industry, veterinarians, and reptile owners is needed to help prevent the risk of salmonellosis associated with animal-based food intended for reptiles.

The Wyoming Department of Health has confirmed a Salmonella outbreak caused by a pig or pigs at the Johnson County Fair.

After a number of Johnson County Fair participants fell ill with stomach cramps and diarrhea, the Department of Health requested stool samples from five people and was able to confirm that all five were suffering from the same type of salmonella.

According to the department’s surveillance epidemiologist Tiffany Greenlee, when two or more people get the same illness from contact with the same animal or animal environment, the event is called a zoonotic outbreak. Greenlee said the pathology reports indicate that the bacteria was transferred from animal to person via pig feces.

USA – Third case of Vibrio confirmed in Mobile County

WKRG

MOBILE, Ala. (WKRG) — The third case of Vibrio in 2018 has been reported to the Mobile County Health Department (MCHD). The case is currently under investigation by MCHD’s Infectious Diseases & Outbreak division.

MCHD says the exposure took place while cleaning crabs in the coastal waters of Mobile Bay. The species has been identified as Vibrio vulnificus. Necrotizing fasciitis — an infection that results in the death of the body’s soft tissue — was present in this case.

Vibrio bacteria naturally live in certain coastal waters and are present in higher concentrations between May and October when water temperatures are warmer. However, the bacteria can be present throughout the year in some areas. While Vibrio bacteria can enter the body through a break in the skin, it can also come from consuming contaminated seafood.

The Alabama Department of Public Health has issued a news release titled “Do not enter bodies of water if you have cuts or abrasions; if injured, clean wound at once to reduce risk of infection.” Here is the link: http://www.alabamapublichealth.gov/news/2018/06/15.html/

Of the more than 70 species of Vibrio that exist, about a dozen can cause human illness — known as Vibriosis. The Centers for Disease Control and Prevention estimates that each year in the United States 80,000 individuals become sick with Vibriosis, and 100 people die from their infection. About 52,000 of these illnesses are estimated to be the result of eating contaminated food.
