Sweden -Sweden reports increase in Cryptosporidium, most in Stockholm

Swedish health officials, aka Folkhalsomyndigheten have reported an increase in the parasitic infection, cryptosporidiosis in recent weeks, particularly in November.

About half of the country’s regions have reported cases in all ages, but primarily in adults. Most cases are seen in the Stockholm region.

The Public Health Agency and the National Food Agency have started an outbreak investigation to identify if there are one or more common sources of infection that are unknown so far. As part of the investigation, cases are interviewed about what they ate and drank before they became ill.

HPS publishes annual surveillance reports for Cryptosporidium, Giardia and Cyclospora in Scotland, 2018

On 17 September 2019, Health Protection Scotland (HPS) published annual surveillance reports for laboratory-confirmed cases of:

  • Cryptosporidium – In 2018, HPS received 536 laboratory reports of Cryptosporidium. This represents an increase of 27 reports (5.3%) when compared with 2017 (509 reports), but is within the year-on-year variation observed in Cryptosporidium as reported in the previous ten years.
  • Giardia – In 2018, HPS received 199 laboratory reports of Giardia, which represents a decrease of 70 reports (26.0%) when compared with 2017 (269 reports).
  • Cyclospora – A total of 12 laboratory reports of Cyclospora were received by HPS in 2018, compared with 46 in 2017, 167 in 2016, 24 in 2015 and two in 2014.

What is already known about this topic?

Cryptosporidium is the leading cause of outbreaks of diarrhea linked to water and the third leading cause of diarrhea associated with animal contact in the United States.

What is added by this report?

During 2009–2017, 444 cryptosporidiosis outbreaks, resulting in 7,465 cases were reported by 40 states and Puerto Rico. The number of reported outbreaks has increased an average of approximately 13% per year. Leading causes include swallowing contaminated water in pools or water playgrounds, contact with infected cattle, and contact with infected persons in child care settings.

What are the implications for public health practice?

To prevent cryptosporidiosis outbreaks, CDC recommends not swimming or attending child care if ill with diarrhea and recommends hand washing after contact with animals.

Research – Toxoplasma gondii and Other Zoonotic Protozoans in Mediterranean Mussel (Mytilus galloprovincialis) and Blue Mussel (Mytilus edulis): A Food Safety Concern?

Mediterranean mussels (Mytilus galloprovincialis) and blue mussels (Mytilus edulis) are among the most consumed fishery products, but they are frequent vehicles of foodborne infection worldwide. In this study, we investigated the occurrence and seasonality of zoonotic protozoans in mussels farmed or sold at retail outlets in Italy. We collected and tested 1,440 M. galloprovincialis and 180 M. edulis. Pooled samples were molecularly tested for Giardia duodenalis, Cryptosporidium spp., and Toxoplasma gondii and then sequenced. Sixty-two (45.9%; 95% confidence interval, 37.5 to 54.3%) mussel pools tested positive for one or more of the investigated pathogens. Both Mytilus species and samples from all the investigated areas harbored pathogens. Mussels were statistically more contaminated by Cryptosporidium spp., followed by T. gondii and G. duodenalis assemblage A, and M. galloprovincialis was more contaminated than M. edulis (P < 0.01). Contamination was more likely in mussels at retail outlets (P < 0.05) than in those from farms and in mussels collected in spring (P < 0.01) than in other seasons. This is the first report of T. gondii found in M. galloprovincialis in Italy and in M. edulis in Europe. The detection of zoonotic protozoans in a widely consumed food source indicates the need for a more detailed microbiological risk analysis, especially considering that bivalve mollusks are often consumed raw worldwide.

Research – Probiotic Product Enhances Susceptibility of Mice to Cryptosporidiosis

ABSTRACT

Cryptosporidiosis, a leading cause of diarrhea among infants, is caused by apicomplexan parasites classified in the genus Cryptosporidium. The lack of effective drugs is motivating research to develop alternative treatments. With this aim, the impact of probiotics on the course of cryptosporidiosis was investigated. The native intestinal microbiota of specific pathogen-free immunosuppressed mice was initially depleted with orally administered antibiotics. A commercially available probiotic product intended for human consumption was subsequently added to the drinking water. Mice were infected with Cryptosporidium parvum oocysts. On average, mice treated with the probiotic product developed more severe infections. The probiotics significantly altered the fecal microbiota, but no direct association between ingestion of probiotic bacteria and their abundance in fecal microbiota was observed. These results suggest that probiotics indirectly altered the intestinal microenvironment or the intestinal epithelium in a way that favored proliferation of C. parvum.

IMPORTANCE The results of our study show that C. parvum responded to changes in the intestinal microenvironment induced by a nutritional supplement. This outcome paves the way for research to identify nutritional interventions aimed at limiting the impact of cryptosporidiosis.

An outbreak of cryptosporidiosis has occurred among people who visited the Shades of Sherwood Campground in Zumbrota, Minnesota. Three people have been sickened in this outbreak; one person has been hospitalized. One of these patients was also infected with Shiga toxin-producing E. coli.

Research – Comparison of three cryptosporidiosis outbreaks in Western Australia: 2003, 2007 and 2011

Cryptosporidium is a protozoan parasite that causes the diarrhoeal disease, cryptosporidiosis. Although many species have been identified, the majority of human disease worldwide is caused by two species; Cryptosporidium parvum and Cryptosporidium hominis. In Australia, data from the National Notifiable Diseases Surveillance System (NNDSS) show that cryptosporidiosis outbreaks occur every few years. To better understand the transmission, trends and nature of cryptosporidiosis outbreaks in Western Australia, epidemiological and genomic data from three cryptosporidiosis outbreaks in 2003, 2007 and 2011 were reviewed. The 2007 outbreak was the largest (n = 607) compared with the outbreaks in 2003 (n = 404) and 2011 (n = 355). All three outbreaks appeared to have occurred predominantly in the urban metropolitan area (Perth), which reported the highest number of case notifications; increases in case notifications were also observed in rural and remote areas. Children aged 0–4 years and non-Aboriginal people comprised the majority of notifications in all outbreaks. However, in the 2003 and 2007 outbreaks, a higher proportion of cases from Aboriginal people was observed in the remote areas. Molecular data were only available for the 2007 (n = 126) and 2011 (n = 42) outbreaks, with C. hominis the main species identified in both outbreaks. Subtyping at the glycoprotein 60 (gp60) locus identified subtype IbA10G2 in 46.3% and 89.5% of C. hominis isolates typed, respectively, in the 2007 and 2011 outbreaks, with the IdA15G1 subtype was identified in 33.3% of C. hominis isolates typed in the 2007 outbreak. The clustering of cases with the IdA15G1 subtype in the remote areas suggests the occurrence of a concurrent outbreak in remote areas during the 2007 outbreak, which primarily affected Aboriginal people. Both the C. hominis IbA10G2 and IdA15G1 subtypes have been implicated in cryptosporidiosis outbreaks worldwide; its occurrence indicates that the mode of transmission in both the 2007 and 2011 outbreaks was anthroponotic. To better understand the epidemiology, sources and transmission of cryptosporidiosis in Australia, genotyping data should routinely be incorporated into national surveillance programmes.

USA – Colorado officials investigate Cryptosporidium, Giardia cases and possible link to Water World

The Colorado state and Tri-County health departments are investigating whether illness is linked to a June 11-13 incident at Water World, during which untreated pond water might have contaminated pools and drinking fountains or might have been used to make food, ice and drinks at the park.

Two people who visited Water World during that time have been diagnosed with Crypto, and one person has been diagnosed with Giardia. Public health agencies are investigating whether these illnesses are associated with their visits to Water World.

Water World resolved the water contamination issue, and there does not appear to be ongoing contamination.
